Parent/Legal Guardian Consent Form

OVERVIEW
Your dependent has applied for services with the Educational Opportunity Center (EOC) The EOC is fully funded by a grant from the Department of Education. The EOC is geared toward supporting low-income families and first-generation students. It is important to track the income brackets of the families we serve to better understand and advocate for our programs and funding resources. For eligibility, we are required to obtain signed documentation to verify low-income status of our participants. Our focus is to help connect participants to college and career resources, as well as GED/high school completion programs.
Note: Your dependency status determines whose information you need to disclose in order to qualify for certain federal programs. The IRS says you are dependent if either, you are younger than 19 years old or are a "student" younger than 24 years old as of the end of the calendar year. A student is anyone who attends a school with "regular teaching staff, course of study, and regularly enrolled student body at the school." For Federal Financial Aid (FAFSA): anyone under 24 is considered a dependent student, unless they are homeless, in a legal guardianship, were in foster care after 13yo, have chid(ren), are married, or are a veteran/currently serving.

For family size, please put all household members that you provide 50% more of their financial support.

I, (parent/legal guardian), authorize the EOC program to secure a copy of my dependent's transcript, standardized test scores, and other reports/records needed as necessary.
I, (parent/legal guardian), understand that the Educational Opportunity Center is a federally-funded program, this document is subject to review by Federal authorities, and I will be held responsible for the certifications made by my signature.
